PANews reported on February 25 that according to CoinDesk, Ethereum's Pectra upgrade was activated on the Holesky test network at 5:55, but according to blockchain data, it was not finalized initially. Finalization means that once a transaction is confirmed and added to a block, it is immutable and irreversible. It is not entirely clear why the test network failed to complete. Ethereum developers are investigating.

This is not the first time an upgrade has not been completed on a testnet. Previously, in January 2024, when developers were testing the Dencun upgrade, the hard fork was not initially completed on the Goerli testnet.
